JPGM WRITECON 2007: A conference on medical writing

Instructions

1. Print in capital letters only.
2. Only one delegate per registration form.
3. A written confirmation will be sent to you on receipt of the registration form and the payment in full. If the payment is received after 15th March, the confirmation may be included in the Conference Pack. Be prepared to show, if necessary, the confirmation letter and the receipt of your payment at the Registration Desk.
4. The registration is on first come first serve basis. Registration will be stopped when the number reaches 250. There will be no spot registration.
5. Cancellation: no refunds will be provided.
6. The conference badge is to be worn throughout the conference.
7. This will be a strictly "no smoking” conference.

8. Mobile phones which ring during sessions will be confiscated.
9. Liabilities: The fees mentioned in this Registration Package do not cover medical care or travel insurances. The Organizing Committee, including the Secretariat, will not accept liability for damages of any nature sustained by participants or their accompanying persons, or loss of, or damage to, their personal property as a result of this Symposium or related events.
10. If you wish to send money via wire-transfer, please request the details by email.
11. Organizers will not be able to provide accommodation or local transportation. Delegates will have to make these arrangements on their own.

Registration Charges

• Registration fee includes Lunch and morning and afternoon tea.

• Payment should be done by demand draft favoring ‘JPGM Conference’ and should be payable at Mumbai. For outstation cheque add Rs. 50.00 (or USD 5.00) extra.
