I would like to thank for the valuable comment from Subramanian [1] on my previous publication. [2] Subramanian et al. raised a concern on the liver effect of phytoestrogen that can be found in soy and further discussed on susceptibility of the individuals. [1] The pros and cons of using soy as nutraceuticals is an important topic for further discussion. There are many considerations for adverse effects of soy including the hepatotoxicity and soy allergy. The hepatotoxicity can be due to many factors as already discussed. For soy allergy, as Subramanian et al. mentioned, it is a common but usually forgotten problem. Sometimes, the clinical manifestation of soy allergy might distract the interest of physician in charge and this can result in misdiagnosis. The good example is the dermatological manifestation. According to a recent report by Jarmila et al., [3] one-third of the patients with atopic dermatitis have occulted soy allergy as an important underlying problem. In addition, the issue on carcinogenesis should be carefully considered. In breast cancer animal model, stimulation of the growth of mammary tumors is reported. [4] Messina and Wu said that "soy foods, and especially isoflavone supplements, are contraindicated for patients with breast cancer and women at high risk of breast cancer. [4]" It is suggested that the in depth study is needed to verify the underlying pathobiology process of breast carcinogenesis due to soy. [5] Although the described problems are not common, they can exist. As Subramanian et al. mentioned, [1] the topic on the negative effect of soy is important and should be educated to all users.
